Baseline & protocol inclusion
Evaluate 60 minutes after the initial ventilator setup. All three criteria must be present to enter the protocol.

ARDS severity & PEEP titration
Tier on the P/F ratio measured with PEEP above 5 cmH₂O, then titrate PEEP to the tier target.
Verify the inclusion criteria to unlock severity tiering.
Plateau pressure & tidal volume
Hold an inspiratory pause on a passive breath, then compare plateau pressure against the 30 cmH₂O ceiling.
Tier severity to unlock the volume limits.
Arterial blood gas & rate optimisation
A pH inside 7.35–7.45 needs no rate change. Otherwise derive the target PaCO₂ from Winter’s formula and work back to a set rate.
Evaluate tidal volume to unlock the rate calculation.
Follow-up gas at 60 minutes
Screen the post-titration gas for refractory hypoxemia before settling into routine surveillance.
Complete the blood gas step to unlock the refractory screen.
